In's and Out's of Home Child Care

Recently, I received a memo saying, that if I would like  get any federal money (through assistance, food and grant programs) I must  have a sign in/out sheet.  
I  created one that would allow for single and multiple children. One that had room for daily in and out times,  plus daily initials and weekly signatures.
Parents also needed to be able  to write their check number and the check amount each week. Checks were than dropped in a locked box in the daycare closet.   If parents paid in cash, they handed  the money to me and  I would write down the amount, date and initial the form.  
I've printed a copy of the following form on card stock for each family.  The forms are placed backwards in a file with the families name.  This allows for privacy.
